Fees + Location + ContactPlease phone 370-1020 to schedule your appointment (do not email). All therapists are opted out of MSP. Treatment fees are to be paid directly by the patient. We accept payment by Visa, debit, cash or cheque. If you are on Premium Assistance, your therapist may be able to reduce your fee by $23. Please bring documentation with you. Extended Medical: If you are covered by an extended medical plan through your employer or that of your spouse, Vitality will provide you with a receipt which you present to your plan administrator for reimbursement. The amount of reimbursement depends on your individual plan. Confirm the requirements and stipulations of your plan before embarking on a treatment schedule. |

Etiquette: Please arrive 10 minutes prior to your appointment time. If you arrive late, your appointment will still end at the scheduled time. Cancellations: Please notify us by phone 24 hours prior to your appointment. Location: Vitality Treatment Centre is located at 302-1842 Oak Bay Avenue at Fell Street (2 blocks west of Richmond Avenue) on the 3rd floor (accessible by elevator).
